Swimmer's Shoulder: Prevention and Treatment

Understanding why swimmers develop shoulder pain and how physiotherapy and TCM can help you get back in the water.

You have been swimming for years, maybe decades. It is your meditation, your cardio, your escape from the demands of daily life. But lately, something has changed. That nagging ache in your shoulder starts during your warm-up laps and lingers for hours after you towel off. You find yourself modifying your stroke, avoiding certain movements, wondering if your swimming days are numbered.

“Swimmer’s shoulder” is a broad label for shoulder pain related to swimming rather than one single diagnosis. Rotator-cuff-related pain is common, but labral, joint, nerve, and neck problems can produce similar symptoms. An assessment helps guide the right plan.

Why Swimmers Are Vulnerable

Swimming demands an extraordinary amount from your shoulders. Competitive swimmers may perform over a million stroke cycles per year, and even recreational swimmers log thousands of repetitive overhead movements. Your shoulder was designed for mobility rather than stability, which means the muscles and tendons working to keep everything in place face tremendous stress with each stroke.

The rotator cuff and shoulder-blade muscles repeatedly control the arm overhead. Pain is rarely explained by “impingement” alone: training load, fatigue, technique, strength and endurance, previous injury, and individual anatomy can all contribute.

Symptoms may begin subtly: stiffness after longer swims, an ache during the catch or pull, or pain when lying on that side. Early load adjustment may stop a mild problem from escalating, but outcome depends on the diagnosis and many individual factors.

The Role of Technique and Training

Poor stroke mechanics amplify shoulder stress considerably. A dropped elbow during the pull phase, inadequate body rotation, or crossing over the midline with your hand entry all increase impingement. Similarly, rapid increases in training volume or intensity—common when preparing for competitions or summer swimming season—can overwhelm tissues that need time to adapt.

Reduced external-rotator or shoulder-blade muscle endurance may be relevant for some swimmers. These findings are not universal causes, so strength testing should be interpreted alongside technique, training load, symptoms, and function.

Rehabilitation usually centres on managing swim volume and rebuilding capacity with progressive rotator-cuff and shoulder-blade strengthening. Manual therapy or acupuncture may provide short-term symptom relief for some people, but neither has been shown to “accelerate” swimmer’s shoulder recovery reliably and neither replaces exercise and load management.

Rehabilitation builds the capacities that testing identifies, often including rotator-cuff and shoulder-blade endurance, trunk movement, and swimming-specific load. Stroke coaching may help when technique changes reduce symptoms.

Preventing Future Problems

Once you have recovered, staying healthy requires attention to several factors:

  1. Warm up thoroughly before swimming, including dynamic shoulder movements
  2. Progress training gradually, changing volume or intensity in steps your shoulder tolerates
  3. Maintain balanced strength with regular exercises for rotator cuff and scapular muscles
  4. Address technique issues with coaching if needed
  5. Listen to your body and reduce training at the first sign of shoulder discomfort
